The form asks for the page or claim URL, what is wrong, and a link to the evidence. Filing it needs a GitHub account.

What happens next

A person reviews every report. When a claim changes, the claim page shows "Corrected [date]: what changed" and keeps the prior verdict visible. The full history is in the public repository.
